One of the most interesting and gross NPCs added to Elden Ring. Shadow of the Erdtree The DLC is Hornsent Grandam. This surly old woman is holed up in a corner of the Shadowland, but doesn’t seem to have a quest if you find her right away. Of course, FromSoftware being the team that they are, there’s more than meets the eye when it comes to this character. It’s a little tricky to understand, but Grandma has a little story to finish that can be missed if you go too far. Here are all the steps and rewards you get for completing them.

Hornsent Grandam is first found in Belurat’s store, Tower Settlement, which is locked and can only be opened with the corresponding key. Once you unlock the door and meet her, she will have a few lines of dialogue but otherwise nothing to do yet.
Once you have defeated the Divine Beast Dancing Lion boss and obtained the Divine Beast Head as a reward, return to Hornsent while wearing the Helmet and talk to him again before continuing and defeating Messmer the Impaler at Shadow Keep.
After talking to her while carrying the giant lion head, she will first cast the Watchful Spirit spell on you. Go to any grace site, sit down to reset the area, then come back and talk to him again to get the Scorpion Stew.
You are now free to defeat Messmer and return to Horsent to speak once more (you no longer have to wear the lion’s head) for some gourmet scorpion stew.
